What insurance plans do you take?

We accept most major insurance providers. To ensure we accept your specific plan, please contact our office directly for verification.

What services do you offer?

Does your office prescribe eye glasses?

We do not prescribe eyeglasses at our office. However, we can provide a referral to an optometrist or ophthalmologist who can assist you with your eyewear needs.

What should I bring to my first appointment?

For your first visit, please bring the following:

  • A valid photo ID

  • Your insurance card

  • Any relevant medical records or test results

  • A list of current medications

  • Completed new patient form

This will help us provide the best care and ensure your visit runs smoothly.

Where can I park?

Metered parking is available on the street (cash and credit cards accepted). There is also a public pay-by-the-hour parking garage one block North from our office called the Summit Garage. The garage entrance is located on Hawthorne Ave.
